Submission of Data and Supporting Documents for Identifying the Ultimate Beneficial Owner

Natural person

If the public sector partner is a natural person, the register records:

  • first name and surname,
  • residence,
  • date of birth,
  • a list of beneficial owners including: first name, surname, permanent residence address, date of birth, nationality, and whether the beneficial owner is a public official holding office in the Slovak Republic,
  • details of the authorised person.

Natural person – sole trader

If the public sector partner is a natural person – sole trader, the register records:

  • business name,
  • place of business,
  • organisation identification number, if assigned, otherwise date of birth,
  • a list of beneficial owners including: first name, surname, permanent residence address, date of birth, nationality, and whether the beneficial owner is a public official holding office in the Slovak Republic,
  • details of the authorised person.

Legal entity

If the public sector partner is a legal entity, the register records:

  • name or business name,
  • registered office,
  • legal form,
  • organisation identification number, if assigned,
  • a list of beneficial owners including: first name, surname, permanent residence address, date of birth, nationality, and whether the beneficial owner is a public official holding office in the Slovak Republic,
  • a list of public officials holding office in the Slovak Republic who are part of the ownership or management structure of the public sector partner,
  • details of the authorised person.

In the case of a public sector partner that is an issuer of securities admitted to trading on a regulated market and subject to disclosure requirements under a special regulation, an equivalent legal regulation of an EU member state or another state party to the Agreement on the European Economic Area, or equivalent international standards, or a company directly or indirectly and exclusively controlled and managed by such an issuer, the statutory body and its members are registered instead of the beneficial owners. In these cases, the verification document must demonstrate that the conditions for registering the statutory body and its members are met. Instead of the permanent residence details of a member of the statutory body, the authorised person may, even without meeting the conditions under paragraph 6, state the registered office of the registered public sector partner.

The register also records the date of registration, the date of each verification of the beneficial owner's identity, the date of registration and deregistration of recorded data, the date on which a deregistration decision becomes final, and the date on which a fine decision becomes final. All verification documents also form part of the register. A public sector partner may have only one authorised person registered at any time.
